Going through the spam comments on one of the sites I found the following:
Yes that’s right… 3 comments on the SAME post from the SAME IP address… how could that not be spam?

Even just 1 comment like that is obviously spam, as it doesn’t refer to anything concrete in the article. Just ‘hey man, that’s cool’ or whatever. All those get dropped on my blog.
Akismet does a good job and after a while it stops – I guess some centralised database works out that nothing makes it through and they give up.
